The purpose of the SFN Early Intervention Survey is to gather information from parents of young children who are receiving or who have recently received Early Intervention services with the objective of enhancing the level of parental engagement of both moms and dads. All responses are anonymous and confidential. For those interested in receiving a copy of the survey results and token gift (a Great Dad Coin), parents can include their name and address.
